DATA OWNER'S EXPLICIT CONSENT DECLARATION FORM

I hereby acknowledge that I have read this Information Notice and, within the scope of the Personal Data Protection Law No. 6698, I consent to the collection, recording, processing, and storage of my personal data by ANVAR AHMEDOV in accordance with the principles of the law, freely and without any pressure or influence. I declare and undertake that, pursuant to the Personal Data Protection Law No. 6698 and the Regulation on the Processing and Protection of the Privacy of Personal Health Data, I consent to the processing of my personal data, including my sensitive personal data, without expanding the scope against my interest and in compliance with the principles of data minimization and purpose limitation, including any personal data that directly or indirectly identifies me, provided by me in verbal/written and/or electronic format, as well as the processing of my health data by ANVAR AHMEDOV for the purposes of candidate selection, signing an employment contract, fulfilling employment contract obligations, and providing and planning health services. I also consent to the sharing of my personal data, sensitive personal data, and health data with other persons and organizations where sharing is necessary under the relevant legislation.
